Could I Still Be Pregnant Despite Having Spotting Along With Negative UPT?

I have had the Mirena birth control for about 3 years. I had intercourse last night and was in no pain right after, nor for the remainder of the evening/night. When I woke up this morning I had extreme cramping that has not subsided and I was bleeding a bright red blood... It s now 9pm and I m still in pain and still bleeding... I took a pregnancy test and it was negative. What could be wrong? Should I visit the Emergency Room?
